Support Coordination

  • Support Coordination assist participant to connect with the NDIS and other services that are specified in their plan.
  • Manage the plan’s budget and broker assistance and services in accordance with participant preferences.
  • The efficiency with assistance in achieving capacity structures, and monitoring plan budgets implementing necessary services selected by participants.
How does Support Coordination work?

Support Coordination has three levels

Level 1 is Support Connection

To make the most of their NDIS plan, participants can use support to help them understand their plan, connect with NDIS providers and community, mainstream, and other resources. Additionally, it ought to give participants more self-assurance and independence in managing their plans.

Level 2 is Coordination of Supports

Support for putting a combination of supports in place to improve a participant’s ability to handle chores, maintain relationships, live more independently, and participate in their community. It increases the participant’s self-assurance and ability to manage their lives rather than just their services.

Level 3 is specialist Support Coordination

Has a higher degree of assistance for people living with disabilities with more complicated situations or require complex or specialised care.

Participants will be assisted to access a range of Allied Health professionals, medical professionals or independent services offered by certified professionals to assist with capacity building.
